The NACOLG/Department of Aging Services administers
the program at the local level, which covers Colbert, Franklin,
Lauderdale, Marion and Winston Counties. NACOLG has three

County & Winston County: (205) 485-7120 TO QUALIFY: 2. Must have no prescription drug coverage 3. Must have a chronic medical condition 4. Must be a legal resident of Alabama 5. Must meet income guidelines Other individuals may be eligible: 6) If you are in the Medicare Part D Coverage Gap 7) Disabled individuals of any age in the 24 month Medicare waiting period A SenioRx Intake form must be completed, as well as, the following information must be available: 1) Proof or all insurance (Blue Cross, QMB, Medicaid, other insurance); 2) A list of your present medicines from your pharmacy, your medication bottles or new prescriptions; 3) Your Social Security Benefits letter for the current calendar year for EVERYONE in the house who receives benefits; 4) Current pension or retirement benefit letters; 5) Proof of income for EVERYONE who lives in your house, including copy of the last three paycheck stubs for each person who is working; 6) Proof of any other income, such as: annuities, stocks, bonds, CDs, savings accounts, IRAs, interest income; 7) A copy of your Medicare Card, Social Security and driver’s license or photo identification; 8) For those disabled, must provide Social Security approval letter. 200% of Poverty Level: 1 Person Living in Household $20,800 2 People Living in Household $28,000 FREQUENTLY ASKED QUESTIONSØ Each drug company has individual requirements for eligibility. SenioRx cannot guarantee that you will receive all medicines requested. Ø Unless you are physically unable, you will be expected to help collect what is needed to complete the process. Ø Most drug companies mail the medicine directly to your doctor. Ø It usually takes anywhere between 6 to 8 weeks after mailing off you application. Please let your SenioRx know when you receive your first medication. Ø You will need to call your SenioRx Office for re-application once you start your last month’s supply of medication. SenioRx staff will verify any medication changes and order the refill prescriptions at that time. Ø All personal information will be kept strictly confidential.
